Mediating effect of self-efficacy between self-regulatory fatigue and exercise adherence in patients with chronic heart failure

Abstract: Objective To explore the mediating role of self-efficacy between self-regulatory fatigue and exercise adherence in patients with chronic heart failure (CHF). Methods A total of 267 CHF patients visiting a tertiary grade-A general hospital in Nanjing were conveniently sampled and surveyed by the Self-Regulatory Fatigue Scale, General Self-Efficacy Scale, and the Exercise Adherence Scale from July to November 2022. Results The total score of self-regulatory fatigue, self-efficacy and exercise adherence in CHF patients was 44.31±8.23, 23.90±6.66 and 29.44±11.59 respectively. Self-regulatory fatigue was negatively correlated with self-efficacy (r=-0.533, P<0.001) and exercise adherence (r=-0.405, P<0.001). Self-efficacy was positively correlated with exercise adherence (r=0.455, P<0.001). The mediation model showed that self-efficacy played a partial mediating role between self-regulatory fatigue and exercise adherence. The total effect was -0.570; and direct and indirect effect was -0.319, and -0.251 respectively. The mediating effect accounted for 44.0% of the total. Conclusion CHF Patients present a high level of self-regulatory fatigue, a medium-low level of self-efficacy, and a low level of exercise adherence. Both self-regulatory fatigue and self-efficacy can directly affect patients' exercise adherence, and self-regulatory fatigue can also indirectly affect patients' exercise adherence through self-efficacy. Strategies aimed at alleviating self-regulatory fatigue and enhancing self-efficacy are advantageous in promoting exercise adherence among CHF patients.
