Abstract: Objective To analyze the influencing factors and pathways of functional exercise adherence of young and middle-aged patients with hemorrhagic stroke, and to provide theoretical basis for medical staff in exercise rehabilitation management of patients. Methods From July 2021 to May 2022, 369 young and middle-aged patients with hemorrhagic stroke in neurosurgery wards of 3 tertiary grade-A hospitals in northern, eastern and central China were collected by convenience sampling method, and they were surveyed by using general information questionnaire, Stroke Health Belief Scale, Exercise Self-Efficacy Scale, Exercise Planning Scale and Stroke Functional Exercise Adherence Questionnaire. Results The total functional exercise adherence score of 369 patients was 43.18±9.57. Functional exercise adherence of young and middle-aged patients with hemorrhagic stroke was positively correlated with health belief, exercise self-efficacy and planning (r=0.529, 0.717, 0.686; all P<0.01). Health belief had a directly positive predictive effect on exercise self-efficacy, planning, and functional exercise adherence (β=0.393, 0.371, 0.155; all P<0.01). Exercise self-efficacy and planning had mediating effect on health belief and functional exercise adherence (β=0.375, P<0.01), with a mediating effect value of 70.75%. Conclusion Exercise self-efficacy and planning play a mediating role between health belief and functional exercise adherence. More health education for enhancing the internal driving force for young and middle-aged patients with hemorrhagic stroke helps to form exercise planning, and then improve patient's functional exercise adherence.

Key words: young and middle-aged, hemorrhagic stroke, health belief, exercise self-efficacy, planning, functional exercise adherence
