Optimization of Intelligent Medication Administration System and Its Application in Elderly Chronic Patients during Transition from Hospitalization to Home

Abstract: Objective To optimize the intelligent medication administration system and evaluate its feasibility in elderly chronic patients during transition from hospitalization to home. Methods The intelligent medication administration system was optimized based on co-design theory and demands analysis. From June to September 2021, elderly chronic patients were selected and randomly divided into intervention group (n=49) and control group (n=45). Optimized intelligent medication administration system was used the intervention group, while routine medication management was performed in the control group. Medication adherence, medication knowledge, and the score of self-efficacy of medication administration were collected and a system evaluation scale was used in the intervention group to analyze the intervention effect. Results The score of medication adherence, and that of self-efficacy of medication administration were higher in intervention group than the control group (P<0.05). There was no statistical significance in medication knowledge score in the patients whether used WeChat mini program or not (P>0.05) and 92% of the subjects were satisfied with the optimized system. Conclusion The optimized system can effectively improve the medication adherence and self-efficacy of medication administration of elderly patients, and it is feasible and practical thus to expand the scope of medication safety, which is worthy to be promoted.
