晚期癌症患者代理决策者安宁疗护决策后悔研究的范围综述

摘要： 目的 对晚期癌症患者代理决策者安宁疗护相关决策的决策后悔研究现状进行分析,为今后相关研究提供指导。方法 采用范围综述的方法,检索PubMed、Web of Science、Cochrane Library、CINAHL、Embase、Proquest、PsyInfo、知网、维普、万方和中国生物医学文献库,检索时限从建库至2023年8月,对纳入文献进行分析和总结。结果 共纳入17篇文献,目前该领域探索的决策后悔研究,其决策内容可归纳为选择安宁疗护类和选择延续生命类2种类别,且普遍讨论结果后悔类型;决策后悔发生率为27.0%~52.7%,受与医生的人际关系和沟通、疾病预后披露、死亡准备状态等因素影响;有4种评估工具,评估时机不固定为患者逝后1~24个月。结论 未来国内需深化各决策后悔类型的影响因素研究,探索干预方案,验证现有国外专科性决策后悔评估量表,拓展开发决策后悔预防性评估工具,将决策后悔纳入决策效果评价指标,为该领域干预性研究提供理论基础,提升对晚期癌症患者代理决策者的照护质量。
