322例老年COPD患者生活空间现状及影响因素分析

摘要： 目的了解老年COPD患者生活空间现状,并分析其影响因素。方法采用便利抽样法,选取2021年11月—2022年4月收治于乌鲁木齐市某三级甲等医院呼吸与危重症医学中心的322例老年COPD患者作为研究对象,采用一般资料调查表、老年人生活空间评价量表、简易躯体能力测试、呼吸困难信念问卷进行调查。运用多元线性回归分析老年COPD患者生活空间水平的影响因素。结果老年COPD患者生活空间总分为（47.10±19.70）分。多元线性回归分析显示,文化程度、病程、呼吸困难指数、躯体功能、呼吸困难信念是老年COPD患者生活空间的主要影响因素（P<0.05）,可解释总变异的44.9%。结论本组老年COPD患者生活空间处于轻度受限,文化程度、病程、呼吸困难指数、躯体功能、呼吸困难信念是影响老年COPD患者生活空间的主要因素。医护人员需重视对生活空间的评估,改善患者躯体功能,降低恐惧呼吸困难信念,最大程度地拓展生活空间。

Abstract: Objective To understand the current status of life space in elderly patients with chronic obstructive pulmonary disease (COPD), and to analyze its influencing factors. Methods Convenience sampling was used to select 322 elderly patients with COPD who were admitted to respiratory and critical care medicine center of a tertiary grade-A hospital in Urumqi from November 2021 to April 2022. General information questionnaire, Life Space Assessment Scale, the Simple Physical Ability Test, and the Breathlessness Belief Questionnaire were used to conduct a questionnaire survey, and multiple linear regression was used to analyze the influencing factors of life space level. Results The life space score of elderly COPD patients was 47.10±19.70. Multiple linear regression analysis showed that education background, disease course, dyspnea index, physical function, and dyspnea belief were the influencing factors of life space in elderly COPD patients (P<0.05), which explaining 44.9% of the total variation. Conclusion The life space of the elderly COPD patients in this group is slightly restricted. Education background, disease course, dyspnea index, physical function, and dyspnea belief are the main factors affecting life space of elderly COPD patients. Medical staff should pay attention to the assessment of life space; improved physical function and reduced fear of dyspnea are conducive to expanding the life space.
