Abstract: Objective To systematically evaluate and integrate the real experience of primary caregivers of adolescents with non-suicidal self-injury (NSSI), to provide strategies for the development of effective interventions. Methods We searched databases including Scopus, Embase, PubMed, The Cochrane Library, Web of Science, CINAHL, CBM, CNKI, Wanfang Data, and VIP to retrieve qualitative studies on the real experience of primary caregivers of adolescents with NSSI from the inception to April 28, 2022. The JBI Critical Appraisal Tool for qualitative studies was used to assess the quality of the literature, and the Meta integration was used to integrate the findings. Results Sixteen articles were collected, extracting seventy results. Three synthesized findings were grouped from seven categories: changes in the internal psychological field; dynamic changes in the external environmental field of caregivers, and changes in the behavior of caregivers. Conclusion The primary caregivers bear physical and mental burden of caring for adolescents with NSSI while facing social and family challenges. Urgent support from hospitals, communities, and families should be considered.
