Risk Factors of Cognitive Impairment after Ischemic Stroke: A Prospective Cohort Study

Abstract: Objective To investigate the incidence of cognitive impairment after ischemic stroke and analyze its influencing factors. Methods Patients with ischemic stroke in two tertiary grade-A hospitals of integrated traditional Chinese medicine and western medicine from December 2020 to December 2021 were enrolled in a prospective cohort study. Patients' data were collected and patients were assessed by using Mini-Mental State Examination (MMSE) 3 months after the stroke. Single factor analysis and multivariate logistic regression analysis were used to analyze the risk factors of cognitive impairment after ischemic stroke. Results A total of 415 patients completed the investigation, and cognitive impairment was observed in 178 cases, accounting for 42.9%. Multivariate logistic regression analysis showed that risk factors for cognitive impairment after ischemic stroke included age (OR=1.094,95%CI: 1.061~1.129), apolipoprotein E (OR=1.320,95%CI:1.108~1.573), homocysteine (OR=1.056, 95%CI: 1.027~1.085), NIHSS score (OR=1.155,95%CI: 1.023~1.305), smoking history (OR=2.408, 95%CI: 1.296~4.475), stroke history (OR=2.932,95%CI: 1.697~5.066), temporal infarction (OR=2.776, 95%CI: 1.094~7.039), qi-deficiency and blood-stasis syndrome (OR=8.694,95%CI: 4.160~18.167), and syndrome of phlegm-heat internal block (OR=4.134,95%CI:1.131~15.111). Conclusion There is high incidence of cognitive impairment after ischemic stroke. Age, apolipoprotein E, homocysteine, NIHSS score, smoking history, stroke history, temporal infarction, qi-deficiency and blood-stasis syndrome, and syndrome of phlegm-heat internal block are in the list of independent risk factors for cognitive impairment after ischemic stroke.
