Abstract: Objective To establish and practise case management model for patients with deep venous thrombosis (DVT) in hospital. Methods DVT case managers were selected and trained and a multi-disciplinary team and DVT reporting system were built. After formulating standards and screening case management objects, DVT health guidelines and case management files were prepared, then case management were implemented. Results All 20 cases finished their treatment without clinical symptoms related to DVT from January to June in 2019, of which 19 patients had good compliance with anti-coagulation treatment and 1 patient nasal hemorrhage. After the guidance of case managers and multi-disciplinary team, less anticoagulant drugs were used and no serious bleeding occurred. Sustained follow-up rate reached 100%. Conclusion Case management model for DVT patients in hospital provides scientific and professional nursing services to patients. Self-management of patients can be strengthened the development of venous thrombus nursing specialty can be promoted.
