Abstract: Objective To develop a scientific and specialized nursing evaluation form of rehabilitation admission for stroke patients, and to explore its clinical application effect.Methods Taking the characteristics of stroke patients and the needs of nursing into consideration,we developed a rehabilitation admission evaluation form for stroke patients after expert consultation using Delphi method and the form mainly consisted of basic information, motor ability, shallow sensory function, swallowing function, balance function, sleep condition, nutrition risk screening, and communication ability. Convenience sampling was used to select stroke patients admitted to the department of neurorehabilitation of Taihe Hospital in Shiyan, and the recognition rate of high-risk patients, the compliance rate of nursing problems, and incidence of adverse events, such as tumble, fall, pressure ulcers, deep vein thrombosis, aspiration, and pyrosis before (January to December 2017) and after (January to December 2018) the application of the developed form was compared.Results After the application of nursing evaluation form, the recognition rate of high-risk patients and the compliance rate of the nursing problem were significantly higher than those before, but the incidence of adverse events decreased (P<0.05).Conclusion Nursing evaluation form of rehabilitation admission for stroke patients is specialized and effective since the general first-time nursing assessment form is inadequate for assessing the condition of stroke patients. The developed form improves the quality of admission care evaluation and further reduce the incidence of adverse nursing events.
