Abstract: Objective To systematically evaluate the stigma and its influencing factors in stroke patients. Methods We searched databases inlcuding The Cochrane Library, PubMed, EMBase, Scopus, Web of Science, CBM, CNKI, Wanfang Data, and VIP from the inception to April 23, 2021. After including observational studies on stigma and its influencing factors of stroke patients, two researchers conducted literature screening and data extraction respectively, and the literature quality was evaluated according to the bias risk assessment criteria of Agency for Healthcare Research and Quality. After qualitative analysis, the general information and results in the included literature were summarized. Results A total of 21 studies were included, with 17 studies in Chinese and 4 studies in English. Twenty-seven influencing factors of age, activities of daily living, economic conditions, social support, with sequelae or not, and depression were summarized into three categories: personal factors, environmental factors, and stroke-related factors. Conclusion Stroke patients generally have a sense of stigma but with inconsistent levels. The main factors affecting stigma of stroke patients are the ability of daily living, economic conditions, social support, with sequelae or not and depression, and the secondary factors are age, occupation, marital status, personality type, coping style (avoidance), and type of disease, number of incidences and number of combined chronic diseases. Other controversial influencing factors need to be further studied and verified.
