Visualization Analysis of Researches on Transitional Care Based on Web of Science

Abstract: Objective To reveal the research and development trend in the field of transitional care through visualization analysis of the relevant literature retrieved from Web of Science. Methods The core collection of Web of Science was searched to obtain literature regarding transitional care published from 2000 to 2019. The retrieved literature was analyzed by CiteSpace software for authors, countries, institutions, journals, co-citations, and keywords. Results A total of 1,186 articles were obtained, and the annual publications began to increase significantly from 2012. Prof. Naylor from the University of Pennsylvania and Prof. Coleman from the University of Colorado are pioneers in the field. The research focuses are the evaluation of the intervention effect on elderly and chronic patients, the construction of adolescent transition program, the application of telemedicine-based interventions, and the optimization of implementation details. Conclusion Transitional care research is in a period of rapid development. Future researches on patients with specific diseases, using high-tech applications and improving program quality may become hotspots.
