Power, Rights and Culture: An Implication from Changes of Medical Decision-making Mode

Abstract: Objective To clarify the historical changes and evolution logic of medical decision-making mode, and to explore an ideal medical decision-making mode. Methods We analyzed and evaluated relevant literature at home and abroad on concept connotation and changes of medical decision-making mode. Results There were three schools in the connotation of medical decision-making in terms of process, result and behavior, but all of them had the problem of unclear decision-making subject. In the history of modern medical development, the medical decision-making mode had undergone three major changes with the cognitive changes of doctors’ rights and patients’ rights, which were paternalistic decision-making model, informed decision-making mode and shared decision-making mode. However, after the spread of modern medicine to China, and the above-mentioned three modes had evolved into Confucian familism because of local culture. Conclusion The ideal medical decision-making mode consists of trusting the authority of doctors, protecting patients' rights, respecting patients’ preferences and local culture in the decision-making process of clinical treatment and nursing programs.
