Mediating effect of organizational trust between nurses' trust in patients and work engagement based on multiple group analysis

Abstract: Objective To explore the mediating effect of organizational trust between nurses' trust in patients and work engagement in hospitals of different levels based on multiple group analysis. Methods From June to October 2022, 1,141 nurses from 35 tertiary, secondary and first-level hospitals in Sanming City in Fujian Province were selected as the study subject by convenience sampling. The nurses were investigated with the general information questionnaire, Nurses' Trust in Patient Scale, Organizational Trust Scale, and Work Engagement Scale. SPSS 26.0 was used to analyze the correlation among nurses' trust in patients, organizational trust and work engagement; AMOS 26.0 to construct the structural equation model to analyze the mediating effect of organizational trust between nurses' trust in patients and work engagement, and multiple group analysis to analyze the differences among the models of hospitals of different levels. Results The score of nurses' trust in patients, organizational trust and work engagement was 41.00 (27.00,48.00), 48.00 (38.00,53.00), and 52.00 (52.00,64.00), respectively. Structural equation model showed that organizational trust had a partial mediating role between nurses' trust in patients and work engagement (β=0.318, P<0.001), accounting for 56.28% of the total effect. Multiple group analysis showed that nurse trust in patients had the highest path impact on organizational trust (B=0.771, P<0.001) and nurses' trust in patients had the highest path impact on work engagement (B=0.289, P<0.001) in secondary hospitals. Organizational trust had the highest effect on the path of work engagement of clinical nurses (B=0.454, P<0.001) in tertiary hospitals. Conclusion The total score of nurses' trust in patients, organizational trust and work engagement is above the average. Nurses' trust in patients can indirectly influence work engagement through organizational trust. There are differences in the path relationships of nurses' trust in patients, organizational trust and work engagement among hospitals of different levels. It is suggested that nursing managers of hospitals of different levels should formulate different measures to create a positive working atmosphere, so as to improve the work investment of the nursing team.
